All established patients have a portal account.  To initiate portal access please follow the directions found in the email. If you elect not to use the portal, please contact the office by phone.

New patients, please call the office directly.

Patient Portal:

As of January 12, 2015 we will be using a comprehensive patient portal, PortalConnect. Our new portal will allow us to integrate all of our online services, creating a simpler and far more reliable way for you to:

  • schedule appointments
  • exchange secure email with us
  • renew prescriptions
  • pay bills online  
  • access your Health information online

You are, of course, free to choose not to enroll in PortalConnect. Please know, however, that ALL of your contact with the office will then need to be by phone. You will not be able to make appointments online and we will not be able to answer questions or communicate about prescriptions by email. 

You will be prompted to set up secure portal access at your next visit. 

 

Appointment Confirmations:

You will receive a confirmation email through our Portal Connect system, as well as a reminder phone call 1-2 business days prior to your appointment

We charge a $50 fee for missed appointments

We are at 80 Maiden Lane, Suite 901. The closest subway stops are:

Are there forms that should be filled out before I come to the office?

New patients or patients who have not been seen in our office in more than 24 months should fill out a Personal History Form PRIOR to coming in.

Please click HERE for a Personal History Form. Please print, fill out, and bring with you on the day of your visit.

Please also plan to come in 15 minutes PRIOR to your appointment time to fill out additional forms needed to process your visit.

 

What do I do if I have an urgent problem?

If you think you are experiencing a true MEDICAL EMERGENCY please call 911 or go to your nearest hospital.

If you have a urinary tract infection, vaginitis, upper respiratory infection, abnormal bleeding or other URGENT problem, please call the office or book an appointment on line. We will make every effort to accomodate you with an appointment within 24 hours (excluding weekends).

 

Can I communicate with my provider by email?

You may communicate with us via your patient portal . Emails are checked daily during business hours. Email communication should be reserved for prescription requests and non urgent questions.

EMAIL COMMUNICATION IS FOR CURRENT PATIENTS ONLY. IF YOU HAVE NEVER BEEN SEEN IN OUR OFFICE, PLEASE CONTACT US BY PHONE.

 

Can we serve as your primary care provider (PCP)?

We are now considered primary care providers by MOST insurance carriers.  PLEASE CHECK WITH YOUR INSURANCE CARRIER TO BE SURE

For those of you ages 18-40 we are now able to see you for your routine physicals, referrals, and most sick visits. We have always been unhappy with the idea that young, otherwise healthy women, need to visit 2 different doctors for relatively simple things.There is no reason why your gyn needs cannot be tended in the same office as your screening cholesterol testing or a check in for a sore throat. For those of you with more serious medical issues or who are over 40, a general internist will need to be part of your medical team. Sick visits and annual exams can be booked by phone or at on line via your patient portal.

 

How do I get a referral to a specialist?

Please provide us with:

  • The name of the provider / practice you want to see AND their provider number assigned by your insurance company
  • Your current insurance carrier with ID number and Group number
  • The specific reason you need to see the specialist

PLEASE REMEMBER THAT YOU ARE RESPONSIBLE FOR DOUBLE CHECKING YOUR COVERAGE WITH YOUR INSURANCE COMPANY.

PLEASE ALSO COME TO EVERY APPOINTMENT WITH YOUR INSURANCE CARD SO THAT WE CAN HELP YOU MOST EFFECTIVELY.

WE DO NOT PARTICIPATE IN MEDICAID OR MEDICARE PLANS, however we are approved prescribers, so your prescriptions and labs will be covered through us.

 

IF WE DON’T TAKE YOUR INSURANCE,  A ROUTINE ANNUAL VISIT IS $125. For other types of visits charges may vary please call us for more specific information.

Please note that your labs will almost always be covered by your existing insurance even if we are not participating. Please check with your insurance company for in-network laboratories.

How do I get my lab results?

May take 7-10 business days to process. If your lab results are normal you will recieve an e-mail to that effect. You will be called with any abnormal results.

 

 How can I find out if preconception genetic testing is covered by my insurance?

We have had significant difficulties with this in the past. We now partner with LabCorp /Integrated Genetics:

How can I request a prescription?

This service is available for CURRENT patients only. PLEASE ALLOW 2 BUSINESS DAYS FOR PRESCRIPTIONS TO FILLED. Requests should be sent through your Patient Portal or fax (855) 832-4139. You may also call the office directly. Please include the following information with your request:

  • YOUR FULL NAME YOUR BIRTHDATE
  • THE DATE OF YOUR LAST ANNUAL EXAM AND PAP SMEAR
  • THE NAME OF THE DRUG YOU NEED
  • HOW MANY REFILLS YOU NEED
  • WHETHER YOU NEED MONTHLY REFILLS OR A 90-DAY SUPPLY (depending on your insurance requirements)
  • YOUR PHARMACY’S ADDRESS & PHONE NUMBER

How do I get my medical records?

Please fax a SIGNED request to (855) 832-4139 which includes:

  • YOUR NAME
  • YOUR BIRTHDATE
  • WHICH RECORDS YOU NEED
  • WHERE WE SHOULD MAIL OR FAX THE REQUESTED INFORMATION

PLEASE ALLOW UP TO 5-7 BUSINESS DAYS FOR REQUESTS TO BE PROCESSED
IF YOU WOULD LIKE YOUR LAB RESULTS EMAILED TO YOU AS A PDF FILE PLEASE INDICATE THAT AS WELL. (We are currently unable to provide your complete medical record including office notes in this form. )

The first 10 pages of your records will be provided to you free of charge by fax/ email. Any additional pages will be mailed to you at a cost of $0.75/page plus postage
