All established patients have a portal account.  To initiate portal access please follow the directions found in the email. If you elect not to use the portal, please contact the office by phone.

New patients, please call the office directly.

 

IMPORTANT NOTICE OF CHANGES:

We are making a major systems change in our office that we anticipate will make all of our online interactions more streamlined and satisfactory.

 As of January 12, 2015 we will be replacing our current systems with a comprehensive patient portal, PortalConnect. Our new portal will allow us to integrate all of our online services, creating a simpler and far more reliable way for you to:

  • schedule appointments
  • exchange secure email with us
  • renew prescriptions
  • pay bills online  
  • access your Health information online

After January 12, we will no longer be using our current email communications (info@ottenheimerhealth.com, convenience@ottenheimerhealth.com, and the Updox patient portal will be discontinued).

We will replace the $100 convenience fee (and $10 fees for prescriptions filled without an appointment, etc) with a single charge of $69/year. The charge will include all convenience fee services, as well as the new online services listed above. This fee will be in lieu of the various fees we have introduced over the years to help cover the costs of working via the systems we used previously.

You are, of course, free to choose not to enroll in PortalConnect. Please know, however, that all of your contact with the office will then need to be by phone. You will not be able to make appointments online and we will not be able to answer questions or communicate about prescriptions by email. Should you decline to enroll in PortalConnect, we will continue to ask you to pay the $10 processing fee for prescriptions filled without an appointment.

We are very excited to introduce PortalConnect and anticipate a much simpler, more streamlined, faster and more satisfying experience.

You will be prompted to set up secure portal access at your next visit. If you who would like access to messaging and online scheduling before your next appointment, please see an upcoming email with instructions to activate your portal from home. You should receive this instructional email before January 12. After that date, you can call our office to receive portal access. We will be happy to help! We look forward to using this new tool to serve you better in 2015.

Are there forms that should be filled out before I come to the office?

New patients or patients who have not been seen in our office in more than 24 months should fill out a Personal History Form PRIOR to coming in.

Please click HERE for a Personal History Form. Please print, fill out, and bring with you on the day of your visit.

Please also plan to come in 15 minutes PRIOR to your appointment time to fill out additional forms needed to process your visit.

 

What do I do if I have an urgent problem?

If you think you are experiencing a true MEDICAL EMERGENCY please call 911 or go to your nearest hospital.

If you have a urinary tract infection, vaginitis, upper respiratory infection, abnormal bleeding or other URGENT problem, please call the office or book an appointment on line. We will make every effort to accomodate you with an appointment within 24 hours (excluding weekends).

 

Can I communicate with my provider by email?

You may communicate with us via your patient portal . Emails are checked daily during business hours. Email communication should be reserved for prescription requests and non urgent questions.

What is the convenience fee?

THIS WILL BE ELIMINATED AS OF JANUARY 12, 2015 FOR PORTAL CONNECT MEMBERS

After considerable investigation of all our options, and with some regret, we have decided to institute fees for some of our supplemental services that you have, heretofore enjoyed free of charge.

Rest assured, you will always be seen for an urgent medical problem, and someone is always reachable by phone for urgent issues without incurring extra fees. Also, be assured that we will continue to schedule our visits in 30-minute increments, rather than the 10-15 minute appointment slots usually allowed by other practices.

You may choose to pay an “a la carte” fee schedule by service (see below). If you choose this option, you will not have access to extended phone or email consultations about problems or abnormal results. We will instead have a staff member call you to make a brief appointment to come in. Email and phone contact will be appropriate only for brief questions, prescription and medical records requests. If you are a patient who only visits us once a year and is otherwise fine, this is clearly the right option.

Alternatively, you may choose to pay a yearly Convenience Fee of $100. This will give you full access to extended email and phone consultation as well as other supplemental services with no separate charges. At the time of payment, you will be given a private email address for communication with us. For visits, copays and deductibles still apply. This is entirely voluntary. We will try to keep the number of visits to the office to a minimum, but bear in mind that office visits for proper evaluation may be unavoidable.

 

We are well aware that you have seen an increase in cost shifting to you in terms of higher co-pays and deductibles, but physicians have not seen a corresponding increase in payments. Net payments to practitioners have remained unchanged for many years, yet overhead costs of medical practice have skyrocketed in terms of real estate, staffing, and malpractice costs. It is for this reason that we now find it necessary to change our practice and seek compensation for the extra services and accessibility heretofore provided free of charge.

 

Can we serve as your primary care provider (PCP)?

We are now considered primary care providers by MOST insurance carriers.  For those of you ages 18-40 we are now able to see you for your routine physicals, referrals, and most sick visits. We have always been unhappy with the idea that young, otherwise healthy women, need to visit 2 different doctors for relatively simple things.There is no reason why your gyn needs cannot be tended in the same office as your screening cholesterol testing or a check in for a sore throat. For those of you with more serious medical issues or who are over 40, a general internist will need to be part of your medical team. Sick visits and annual exams can be booked by phone or at on line (www.ottenheimerhealth.com or www.jrehnstrommd.com).

 

How do I get a referral to a specialist?

Please provide us with:

  • The name of the provider / practice you want to see AND their provider number assigned by your insurance company
  • Your current insurance carrier with ID number and Group number
  • The specific reason you need to see the specialist

IF WE DON’T TAKE YOUR INSURANCE,  A ROUTINE ANNUAL VISIT IS $125. 
Please note that your labs will almost always be covered by your existing insurance even if we are not participating.
For other types of visits charges may vary please call us for more specific information.
 
 In addition,  labs are generally covered by insurance, even if the doctor’s visit is not, and we will be sure to use an in-network laboratory.

How do I get my lab results?

May take 7-10 business days to process. If your lab results are normal you will recieve an e-mail to that effect. You will be called with any abnormal results.

How can I request a prescription?

This service is available for CURRENT patients only. PLEASE ALLOW 2 BUSINESS DAYS FOR PRESCRIPTIONS TO FILLED. Requests should be sent through your Patient Portal or fax (855) 832-4139. You may also call the office directly. Please include the following information with your request:

  • YOUR FULL NAME YOUR BIRTHDATE
  • THE DATE OF YOUR LAST ANNUAL EXAM AND PAP SMEAR
  • THE NAME OF THE DRUG YOU NEED
  • HOW MANY REFILLS YOU NEED
  • WHETHER YOU NEED MONTHLY REFILLS OR A 90-DAY SUPPLY (depending on your insurance requirements)
  • YOUR PHARMACY’S ADDRESS & PHONE NUMBER

How do I get my medical records?

Please fax a SIGNED request to (855) 832-4139 which includes:

  • YOUR NAME
  • YOUR BIRTHDATE
  • WHICH RECORDS YOU NEED
  • WHERE WE SHOULD MAIL OR FAX THE REQUESTED INFORMATION

PLEASE ALLOW UP TO 5-7 BUSINESS DAYS FOR REQUESTS TO BE PROCESSED
IF YOU WOULD LIKE YOUR LAB RESULTS EMAILED TO YOU AS A PDF FILE PLEASE INDICATE THAT AS WELL. (We are currently unable to provide your complete medical record including office notes in this form. )

The first 10 pages of your records will be provided to you free of charge by fax/ email. Any additional pages will be mailed to you at a cost of $0.75/page plus postage
